Effect of Multidisciplinary program on Pain and Sleep Disorder regarding Patients with Polytrauma

Abstract:
Polytrauma is defined as the condition of a person who has been subjected to multiple traumatic injuries, such as a serious head injury in addition to a serious burn. Multidisciplinary pain management programs based on the bio-psycho-social model are an important tool in the treatment of chronic non-cancer pain. The programs have been proven to be effective for the treatment of pain and disturbed sleep and are also recommended by the current German guidelines for the treatment of low back pain. Common features of all multidisciplinary treatment programs are the high treatment intensity. Aim: The aim of study was to evaluate the effect of multidisciplinary program on pain and sleep disorder regarding patient with polytrauma. Design: A quasi experimental research design was used in this study. Methods:Pre- post assessment of outcome was used in this study. Setting: The study was conducted in surgical units at Al-Ahrar Hospital, Ministry Of Health, Zagazig Governorate, Egypt and new surgical Zagazig University hospital. Sample: A purposive sample of 60 patients with pollytrauma was included. The multidisciplinary program consisted of theoretical and practical part implemented through 20 sessions and lasted for nine months. Tools included sections for Patient Interviewing assessment questionnaire, Specific pain Anxiety Scale"SPAS",Sleep Scale, Pain Behavior Scale "UAB"; in addition to relaxation techniques "RT" Checklist. Results: There was a highly statistically significant difference between pre / post program phaseall of study subjects had unsatisfactory knowledge level in pre-program phase, all (100%) of patients in post program phase had satisfactory knowledge level. There were significant higher mean values of sleep quality in both study and control group at 3rd week. Half of patient in study and control group had mild pain in the third week. Conclusion: Multidisciplinary program had a positive effect on pain reduction and sleep quality among patients with Polytrauma. The study was recommended to integrate multidisciplinary program as a part of routine nursing care along with pharmacological studies for management of pain and sleep disorder.
