The current study aimed to assess the impacts of feeding with Spirulina platensis (SP) and Salvia officinalis (SO) for 28 days on the immune status of Nile tilapia (Oreochromis niloticus). A total of 180 Nile tilapia (30.6 ± 0.12 g) were divided into 3 groups (triplicates of 20 fish/group). Control group was fed on a basal diet only, while the other two groups were supplemented with SP (10 gm/kg diet) and SO (7.5 gm/kg diet) respectively. Both of SP and SO showed in vitro antimicrobial activity against Pseudomonas aeruginosa. The immune response of Nile tilapia supplemented with SP and SO revealed significant increases in lysozyme, nitric oxide activities and IgM titer with enhancement of IL-1β and TNF-α genes expression before and after infection with P. aeruginosa. Fish challenged with P. aeruginosa showed signs of infection with high mortality rate and low relative percentage survival which elevated to be 83% in SP group and 75% in SO group. It could be concluded that dietary supplementation with SP and SO improved immune response and protected Nile tilapia against infection. Moreover, SP showed better results than SO before and after infection.

Statistical Modelling of Categorical Outcome with More than Two Nominal Categories Fatma D.M. Abdallah* Department of Animal Wealth Development, Faculty of Veterinary Medicine, Zagazig University, Egypt *Corresponding author: Nour_stat2013@yahoo.com Received August 13, 2018; Revised October 04, 2018; Accepted December 04, 2018 Abstract This paper aims to explain and apply an important statistical method used for modelling categorical outcome variable with at least two unordered categories. Logistic regression model especially multinomial logistic type (MNL) model is the best choice to model unordered qualitative data. A simulation study was done to examine the efficiency of the model in representing categorical response variable. Three explanatory variables (age, species, and sex) are used for discrimination. While the outcome variable was Rose Bengal Plate Test (RBPT) results which has four outcome categories (negative, positive, false positive, and false negative). Therefore, logit model will be utilized to model this data. MNL models were fitted using SPSS packages and parameters estimated depending on maximum likelihood (MLE) by the Newton-Raphson algorithm. This model depends mainly on two estimates to interpret the results, they are the regression coefficient and the exponentiated coefficients which known as the odds ratio. This model was a good fitted for description the data of 500 values of Rose Bengal Plate Test results of Brucella in sheep and goat species. The results showed fitting of the model to the data with highly significant likelihood ratio statistic for the overall model (P value = 0.000**). Wald test was significant for all variables in positive category and this indicated that age, species and sex are good predictors for test results. The odds ratio in case of positive category for age, species and sex was 1.589, 0.214 and 0.133 respectively. Using Discriminant Analysis and Artificial Neural Network Models for Classification and Prediction of Fertility Status of Friesian Cattle Eman A. Abo Elfadl1,*, Fatma D. M. Abdallah2 1Department of Animal Husbandry and Development of Animal Wealth, Faculty of Veterinary Medicine, Mansoura University, Egypt 2Department of Animal Wealth Development, Faculty of Veterinary Medicine, Zagazig University, Egypt *Corresponding author: emmy_f1984@yahoo.com Abstract Background & objectives: This study was undertaken to compare the accuracies of Discriminant analysis model (DA) and Artificial neural networks model (ANN) for classification and prediction of Friesian cattle fertility status by using its reproductive traits. Methods: Data was collected through field survey of 2843 animal records of Friesian breed belongs to El Dakhalia province farms, Egypt. Data was covering the period extended from 2010 to 2013. The samples of dairy production sectors were selected randomly. Data was collected from valid farm records or the structured questionnaires established by the researcher. Results: The results of classification accuracy indicated that the artificial neural network (ANN) model is more efficient than the discriminant analysis (DA) model in expressing overall classification accuracy and accuracies of correctly classified cases of fertility status for Friesian cattle. The results showed that The ANN models had shown the highest classification accuracy (93.6%) for year (2010) while, it was (79.9%) for DA. The comparison of overall classification accuracies clearly favored the supremacy of ANN over DA. The results also were confirmed by the areas under Receiver Operating Characteristic Curves (ROC) captured by ANN and DA. ROC curves are used mainly for comparing different discriminating rates. Areas under ROC curves were higher in case of ANN models across the different years compared to DA models. The differences in accuracies were also significant at 5% level of significance with p-value 0.005 by using Paired Sample t-test. From all of the above we can conclude that artificial neural network model was more accurate in prediction and classification of fertility status than a traditional statistical model (Discriminant analysis). Center for Emerging Infectious Diseases, School of Medicine, Gifu University, Gifu, Japan Abstract Shiga toxin-producing Escherichia coli (STEC) are group of E. coli causing bloody diarrhea. The goal of this survey was to determine the prevalence of multidrug resistant shiga toxin-producing E. coli in cattle meat and its contact surfaces. Swab samples (n=120) were randomly collected from meat and contact surface of butchery shops in Sharkia province, Egypt. Prevalence of E. coli was examined using culture, biochemical, and serological methods. Identification of shiga toxin-encoding genes (stx1 and stx2) in the E. coli serotypes was done using multiplex polymerase chain reaction. Screening of multidrug resistance profile was done using the disk-diffusion method. Data were analyzed using JMP statistical package, SAS Institute Inc., Cary, NC. The prevalence rates of E. coli in the chuck, round, masseter muscles, cutting boards, walls, and floors were 20, 10, 30, 50, 40, and 60%, respectively. Among the isolates, E. coli O111:H4 and E. coli O26:H11 harbored the two mentioned genes. E. coli O86 and E. coli O114:H21 harbored only stx1; while E. coli O55:H7 encoded only stx2. Just E. coli O124 had no express of stx1 and stx2. The isolated E. coli serovars showed a multidrug resistance profile. Considering the results of this study, strict hygienic procedures should be followed to avoid or reduce carcass cross contamination. THE EFFECT OF ROSEMARY EXTRACT AND LACTIC ACID ON THE QUALITY OF REFRIGERATED BROILER FILLETS Mohamed A. Hussein1, Waleed R. El-Ghareeb1,2 and Mohammed A. F. Abstract The current study investigated the effect of rosemary extract (0.2% RE) and lactic acid (1% LA) on some freshness, bacterial parameters and formation of biogenic amines during refrigeration of broiler fillets. Ninety broiler fillet samples were divided into three groups (each 30). The control group was dipped in sterile distilled water, while the RE and LA groups were dipped in rosemary extract 0.2% (w/v) and lactic acid 1% (w/v), respectively. Sensory evaluation, pH, total volatile basic nitrogen (TVB-N), thiobarbituric acid reactive substances (TBARS), total psychrotrophic and Enterobacteriaceae counts were performed at zero time and at 3 days interval until the group were rejected from sensory analysis. Organoleptic scores were unacceptable after the 9th, 12th and 15th day of refrigeration in control, RE and LA groups, respectively. TVB-N was the lowest in LA group (3rd day; 4.36 ± 0.12, 6th day; 5.62 ± 0.7 and 9th day; 10.13 ± 0.98) when compared with the RE and control groups. Moreover, TBARS was the highest in the control group (3rd day; 0.22 ± 0.06, 6th day; 0.39 ± 0.07 and 9th day; 0.78 ± 0.09) when compared with the RE and LA groups. LA group showed the lowest number of psychrotrophic bacteria during refrigeration period when compared with the other groups. Enterbacteriaceae was detected at zero, 3rd and 6th day in control, RE and LA groups, respectively. This study concluded that dipping in LA 1% or RE 0.2% reduce the psychrotrophic and Enterobacteriaceae bacteria of broiler fillet and increased the acceptability of refrigerated fillet, decreased the biogenic amines formation and rancidity. Therefore, broiler fillets dipped in RE 0.2% or LA 1% can be used up to 12 or 15 days when refrigerated at 4°c C. مكان النشر: Journal of Food Science and Technology; 55 (12): 5025–5034 (2018).

Clinicopathological and Prognostic Value of Immunohistochemical Expression of CD44 (Stem Cell Marker) and Ki67 in Serous Ovarian Cancer Taiseer R. Ibrahim, Samar M. Abdelraouf, Mohamed Abdelgawad, Amira Elwan Abstract Introduction: Failure of effective treatment of epithelial carcinoma of the ovary necessitates searching factors that lead to resistance to chemotherapy and recurrence. Cancer stem cell theory hypothesis provides an explanation of how small group of cells inside the tumour remain resistant to treatment. CD44 has been considered as cancer stem cell marker in multiple malignancies but its role in Ovarian Serous Carcinoma (OSC) is still controversial. Aim: To investigate the expression of stem cell marker CD44 and proliferative activity using Ki67 immunostaining in serous ovarian carcinoma and their relation to clinical and pathological variables and outcome of the patients. Materials and Methods: The present study was a prospective study done on 45 patients with serous ovarian tumours, the ovarian sections obtained from the patients were immunostained by CD44 and Ki67 immunohistochemical markers. The Overall Survival (OS), Disease Free Survival (DFS) and response to treatment were investigated and analysed. Results: Both CD44 and Ki67 were positive in patients with increased grade and advanced stage and the difference was highly statistically significant (p=0.001 and p<0.001, respectively). CD44 and Ki67 expression were higher in bilateral cancer with presence of lymph node and distant metastasis than in unilateral tumours without lymph node and distant metastasis. The present study found positive relation between CD44 and Ki67 expression (p=0.006). CD44 and Ki67 positive expression level in OSC had a significantly shorter 3-year DFS than the patient group with negative expression of CD44 and Ki67 (p=0.020 and p=0.002, respectively). Conclusion: This study demonstrated that CD44 and Ki67 expression were significantly associated with high grade and late stage serous ovarian carcinoma, and their high expression was associated with poor prognosis and resistance to chemotherapy, so proper quantification may play a vital promising function in choosing targeting therapy and predicting more better result outcome.

Expression of Stem Cell Marker OCT4 and Epidermal Growth Factor Receptor Predict Recurrence in Transitional Cell Carcinoma of Bladder in Egyptian Patients Mona M. Ahmed ,Taiseer R. Ibrahim, Safa A.Balata, Mohamed Malek, Ibrahim Ismail, Ayman Omar Abstract Introduction: In the past, the prognosis of bladder cancer was dependent on the presence of muscle invasion. Recently High-Grade T1 (HGT1) Transitional Cell Carcinoma (TCC), found to be associated with similar behaviour and prognosis even following Bacillus Calmette Guerin (BCG) therapy. Recognition of new markers that may help in selection of high-risk cases that need aggressive treatment even in the early stage to avoid unwanted toxicity of chemotherapy has to be high priority. Aim: To evaluate the prognostic value of Octamer-Binding Transcription Factor 4 (OCT4) and Epidermal Growth Factor Receptor (EGFR) immunohistochemical expression in urothelial carcinoma of the bladder and their effect as well as the vascular invasion on the recurrence of these tumours. Materials and Methods: Prospective cohort study conducted in Zagazig University hospital from January 2010 till December 2013. Immunohistochemical staining of OCT4, EGFR and Cd34 for 68 cases of bladder cancer samples was done. The correlation between the expressions of each marker and clinic-pathologic parameters was then analyzed. Results: OCT4 was detected in 53 patients (77.9%) of total cases. EGFR showed high expression in 25 of cases. 18 and 25 cases revealed low and no EGFR expression, respectively. There was a significant association between intensity of OCT4 expression and tumour grade (p<0.001), muscle invasion (p<0.001), vascular invasion (p=0.004) and stage (p=0.001). Significant association was established between EGFR expression and tumor size, grade, lamina propria, muscle and vascular invasion and stage (p 0.005, <0.001, 0.001, <0.001, respectively). Significant association observed between OCT4, EGFR expression and vascular invasion and recurrence (p=0.026, 0.001 and 0.002 respectively). Conclusion: Using selective inhibitors for EGFR and OCT4 especially that approved for EGFR in treating selected cases of urothelial carcinoma of bladder could suppress tumor growth. These markers in addition to presence of vascular invasion have to be used in screening of high risk noninvasive group that benefits from target therapy

ABSTRACT Background: Chylothorax is a rare complication after adult cardiovascular surgery. Chylothorax directly affects postoperative course and morbidity. Methods: Data for this prospective study was collected between July 2017 and August 2021. Twenty patients who had adult cardiac surgery were included in our study. Demographic characteristics, operative and postoperative data were collected and analyzed. Somatostatin injections were used to control chyle leakage. Failure of conservative treatment was the indication of surgery. Results: This study included 20 patients, 13 males and 7 females, who recently underwent adult cardiovascular surgery. The mean age was 47.7±13.27 years. The mean duration for chyle effusion was 4.35±1.98 days, while the mean duration of drainage was 12.15±2.56 days. Analysis of pleural fluid revealed mean pH value = 7.3±0.05, mean total protein =2.7±0.3 g/dL, mean pleural glucose concentrations =123±33 mg/dl, mean cholesterol level =43±17 mg/dL, mean triglyceride level =489±39 mg/dL, and mean chylomicron level =7±0.4 mg/dl. There was a statistical significance regarding the response of postoperative chylothorax to somatostatin therapy (p=0.001). Surgery (mass ligation) was indicated in 4 cases secondary to failure of conservative treatment. There was a significant difference between ICT drainage/day in patients who were managed conservatively and those who were managed surgically (p =0.0001). Conclusion: Chylothorax is more common following CABG and Aortic arch surgeries, and it has a significant impact on the postoperative course and morbidity. The diagnosis and therapy should be initiated as soon as possible. Conservative management is feasible in 80% of cases.

ABSTRACT Background: Nowadays, video-assisted thoracoscopic surgery (VATS) is the management of choice for primary spontaneous pneumothorax (PSP). This prospective study was designed to address the efficacy and the possible advantages of using uniportal VATS when compared with triportal VATS in management of PSP. Method: A total of 80 VATS procedures were randomly done for PSP. 40 procedures were performed with uniportal VATS compared to 40 procedures performed with triportal VATS in the management of PSP. Patients were followed-up for 12 months postoperatively. Results: Both uniportal and triportal VATS have the same efficacy in managing PSP. There is no difference between both techniques regarding postoperative bleeding and air-leak (p=1.0). Uniportal VATS has a shorter operative time (66.32±3.46 vs 72.95±6.08 minutes, p<0.001). Adding another port wasn’t needed in uniportal VATS, while it was needed in 6 (15%) patients in triportal VATS group (p=0.026). Uniportal VATS has a statistically significant difference regarding postoperative pain duration (3.80±1.04 vs 4.32±0.47 days, p=0.005), early postoperative pain scale by visual analogue pain scale (1.77±1.02 vs 2.37±1.25, p=0.022), chest tube duration (4.85±0.57 vs 5.07±0.26 days, p=0.029), postoperative paresthesia (0.05±0.22 vs 0.90±0.74, p<0.001), postoperative hospital stay (5.45±0.50 vs 5.87±0.75 days, p=0.004), and better cosmesis (2.35±0.62 vs 1.92±0.65 on scale 1-3, p=0.004). Conclusions: Uniportal VATS is a safe and a feasible approach in management of PSP. Uniportal VATS is a good alternative to triportal VATS due to its efficacy in decreasing postoperative pain, paresthesia, total duration of hospital stay and in providing better cosmesis.

Abstract Background: There are different techniques for thoracoscopic sympathectomy for management of primary hyperhidrosis. Each technique has the advantage of giving good surgical results, but may cause complications, which depend on the level of sympathectomy used in each technique. Methods: From October 2012 to August 2016, a total number of 120 patients (of primary palmar and axillary hyperhidrosis) were enrolled in this prospective randomized study. Patients were randomized preoperatively and divided into 2 groups: group A (underwent T2, T3, and T4 sympathectomy) and group B (underwent T3 and T4 sympathectomy) with 1 year of follow-up. Improvement of palm sweating, development of complications, patient satisfaction, and quality of life were evaluated. Results: Family history was positive in 33 patients (55%) in group A and in 31 patients (52%) in group B. All patients had improvement of hand sweating. Overdry hands were significant in group A (p = 0.032). Compensatory sweating was significant in group A after the first month (p = 0.016), after 6 months (p = 0.022), and after 12 months (p = 0.025). In group B, very satisfied patients were significant after 6 and 12 months (p = 0.002 and 0.000, respectively). Conclusions: Limited sympathectomy to the level of T3 and T4 has the same therapeutic effect obtained by T2, T3, and T4 sympathectomy with less complications, greater patient satisfaction, and better quality of life. It should be the technique of choice for treatment of primary palmar and axillary hyperhidrosis.

ملخص البحث معلومات أساسية. هناك تقنيات مختلفة لاستئصال التعاطف thoracoscopic لإدارة hyperhidrosis الأولية. كل تقنية لها ميزة إعطاء الخير النتائج الجراحية، ولكنها قد تسبب مضاعفات تعتمد على على مستوى استئصال التعاطف المستخدم في كل تقنية. الأساليب. من أكتوبر 2012 إلى أغسطس 2016، ما مجموعه عدد 120 مريضا (من الرمح الأولي والإبطين فرط التعرق) في هذه الدراسة randomized المستقبلية. تم إجراء عمليات عشوائية للمرضى قبل الجراحة وتنقسم إلى مجموعتين المجموعة ا : (خضعت لـ T2, T3, T4 sympathotomy) ـ T3 و المجموعة ب : T4 sympathomy) مع 1 سنة من المتابعة. Improvement تعرق النخيل وتطور المضاعفات وتم تقييم رضا المرضى ونوعية الحياة.

ABSTRACT Background: Chromium is generated as a waste product from different industries. Its reproductive toxicity has been addressed in many studies that considered oxidative stress the main contributing factor for such toxicity. Selenium is essential for male reproductive health and crucially involved in regulation of cellular redox status. Aim: To elucidate the potential impacts of potassium dichromate (PDC) on the structure of the epididymis, prostate and seminal vesicles and to investigate the efficacy of selenium on counteracting these impacts. Methods: Thirty-six adult male rats were assigned into four groups: control, selenium, PDC, and PDC+selenium. The animals were subjected for a treatment period of four weeks. At the end of experimental period, blood and semen samples, epididymides, prostates and seminal vesicles were collected and processed for biochemical, morphological and immunohistochemical analyses. Results: PDC administration significantly deteriorated sperm parameters and triggered alteration of cellular redox homeostasis, evidenced by increased serum malonaldehyde levels with decreased enzymatic activity of the antioxidants: superoxide dismutase and catalase. Furthermore, PDC significantly unregulated the expression of the serum inflammatory markers; nuclear factor-kappa B (NF-κB), tumor necrosis factor alpha and interleuken-1beta. Microscopically, all the examined tissues of PDC-treated rats displayed deteriorated microarchitecture as well as elevated 8-hydroxy-2-deoxyguanosine and microtubuleassociated protein light chain3 immunoexpression, indicating increased oxidative DNA damage and autophagy activity. Selenium supplements to PDC-treated rats effectively alleviated all the tested parameters. Conclusion: Selenium supplements could effectively mitigate PDC-induced damage of the epididymis, prostate and seminal vesicles through counteracting oxidative stress, and reducing NF-κB activation and excessive autophagy evoked by PDCSELENIUM COULD ALLEVIATE POTASSIUM DICHROMATE-INDUCED EPIDIDYMAL, PROSTATIC AND SEMINAL VESICLE CHANGES IN ADULT RATS: POTENTIAL ROLE OF INHIBITING NF-ΚB PATHWAY

BACKGROUND: Gentamicin is widely used broad spectrum aminoglycoside antibiotic. Despite its beneficial effects, gentamicin has considerable nephrotoxic effects. Salicylic acid is a phenolic compound that has anti-inflammatory, analgesic and antipyretic effect. It has also antioxidant activity that can counteract oxidative damages induced by adverse conditions in animals. OBJECTIVES: The work aims to detect the harmful changes in the rat kidney that occur after administration of gentamicin and to determine whether salicylic acid treatment attenuates gentamicin - induced nephrotoxicity. MATERIAL AND METHODS: Fifty healthy adult male Wister albino rats were obtained from the breading animal house, Faculty of Veterinary Medicine, Zagazig University. These animals were divided randomly and equally in to five groups. Each group contains ten rats. Group 1: Included control animals. Group 2: Included salicylic acid-treated animals. Group 3: Included gentamicin-treated animals. Group 4: Included animals treated with salicylic and gentamicin at the same time. Group 5: Included animals treated with gentamicin and remain for recovery. The rats were anesthetized with ether inhalation and sacrificed. The kidneys were removed and processed for light and electron microscopic examination. RESULTS: The histological study revealed that gentamicin causes severe nephrotoxicity. Exposure to gentamicin caused necrosis of tubular epithelial cells. The glomeruli were also affected. On the other hand, salicylic acid administration protected kidney tissue against the nephrotoxic effect caused by gentamicin treatment. Recovery from gentamicin treatment could occur spontaneously but it was slow and incomplete. CONCLUSION: Gentamicin administration leads to harmful changes in the structure of the rat kidney. Spontaneous recovery could occur but it is slow and incomplete. These nephrotoxic changes can be protected by co-therapy with salicylic acid

Background: The cerebellum is the second largest part of the mammalian brain. It is a key regulator of coordinated sensory and musculoskeletal actions. The cerebellar cortex has a striking morphology consisting of folia and fissures and a variety of cells which are morphologically and functionally different, so it’s considered as an ideal model to study the development of the central nervous system in the mammals. The Objectives: The aim of this work was to study the prenatal and postnatal development of the albino rat cerebellar cortex considering its structure and maturation. Material And Methods: 35 healthy, non-pregnant female and 18 male albino rats weighing (200-250 g) were obtained from the animal house, Faculty of Medicine, Zagazig University. After mating and pregnancy, the rat embryos and offsprings were divided into 3 groups and 6 subgroups; Group A (11th, 16th&20th fetal rats), Group B (7th, 14th &21st postnatal days rats) and Group C (3 months old rats). The fetal rats (11th and 16th) were fixed as whole, while the remaining prenatal, postnatal and adult rats were dissected to obtain cerebella, which were processed for light and electron microscopic examinations, morphometric and immunohistochemical studies. Results: Albino rats at prenatal day 11 (E11) showed formation of the neural tube from the ectodermal layer. Albino rats at prenatal day 16 (E16) showed a demarcated cerebellar anlage (rhombic lip ( at the dorsal part of the metencephalon. The cerebellar cortex at prenatal day 20 (E 20) showed appearance of the general architecture. From outwards to inwards, it consisted of external granular, molecular, Purkinje cell, internal granular layers. Thickness of the external granular layer was maximum at postnatal day 7 (D7) then gradually decreased with age till disappearance in adult age. The molecular layer showed a gradual increase in thickness with age till reaching its maximum size at adult stage. Purkinje cells at (E20) and (D7) were not arranged in a definite layer. It appeared as a single layer between the molecular and internal granular layer at D14, D21 and adult. The internal granular was consisted of scattered small granule cells at E20 and D7. At D14 the internal granular layer became well differentiated and distinguished from the white matter. Immunohistochemistry showed a negative reaction for glial fibrillary acidic protein (GFAP) at E11 and E16. The first positive reaction for GFAP appeared at E20 then gradually increased till adult group. Conclusion: The study concluded that the cerebellar cortex undergoes differentiation and maturation during late prenatal and early postnatal ages early postnatal ages till the third week of life, corresponding to nearly the seventh postnatal month in human. Egyptian Journal of Anatomy (EJA): volume 41 (1), January 2018 ABSTRACT Background: Coarctation of the aorta accounts for 6·8% of congenital heart disease, with an incidence of one in 12 000 live births . Coarctation is a heterogeneous lesion with variability in the degree and site of obstruction. This anatomical variation has an embryological reflection. Up till now, there is no sufficient data regarding the effect of this anatomical component of coarctation of the aorta on the short and term-long prognosis after its treatment (surgical or percutaneous angioplasty). This relationship has to be clarified. Aim of the work: We aim to demonstrate the relationship between the anatomical features of the aortic arch in case of aortic coarctation and the prognosis on the short and long term after its surgical or percutaneous repair. Patients and methods: We conducted a retrospective study that continued prospectively till the end of the work. The children with aortic coarctation who were repaired surgically or percutaneously by catheter-based angioplasty or stenting in the period between 12/2014 and 06/2016 were enrolled. All the descriptive data about this coarctation was collected (echocardiography, CT scan, and/or MRI). An anatomical analysis of these data with their embryological background was done. We have then classified these patients according to their anatomical features of the aortic arch. Follow-up data on short and long-term post-treatment of each anatomical class was gathered. Statistical analysis of these data was held to find out the relationship between this anatomical variation and the follow-up results. Results Thirty cases were enrolled in this study. 15 cases were treated surgically. the other 15 cases were treated interventionally using a balloon or stent. The mean age ± SD was17±12.2 months, the mean systolic blood pressure ±SD pre-treatment was180 ±25 mmHg. 16 cases have hypoplastic aortic arch.13 cases have abnormal aortic arch anatomy (5 have a common trunk of innominate and left common carotid,2 cases have hypoplastic left displaced left subclavian artery, 3 cases have separate right common carotid trunk from the aortic arch, 3cases have a gothic arch with very close aortic arch branches. We found that all cases with hypoplastic aortic arch had recurrent coarctation 6-9m after management. Cases with abnormal arch anatomy had a worse prognosis regarding the persistence of hypertension. There was a negative correlation between the distance (left common carotid and left subclavian arteries) and the duration before coarctation. Conclusion: Arch anatomy may affect the clinical prognosis of coarctation. Abnormal arch anatomy has a worse prognosis. The less the distance between the left subclavian and left common carotid, the better the prognosis and less incidence of re-coarctation. International Immunopharmacology 2023; Volume 115: 109621 Background Ulcerative colitis (UC) is a global inflammatory bowel disease. Aime :This study aimed to assess the effects of icosapent ethyl on acetic acid-induced colitis in rats as well as the underlying mechanisms involved. Methods 36 male Wister rats were equally divided into Six groups: control, UC, mesalamine 100 mg/kg, icosapent 150mg/kg, icosapent 300 mg/kg, and EX527-icosapent 300 mg/kg groups. EXcept for control group, UC was induced by acetic acid instillation into colon. Drugs were administered once daily for one week then under thiopental anaesthesia, colons were excised. Colitis macroscopic and microscopic scores were assessed. A part of colon was homogenized for detection of malondialdehyde (MDA), inerleukin1 (IL-1β), tumor necrosis factor (TNF-α), superoxide dis-mutase (SOD), phosphorylated Akt (pAkt) and caspase 3 levels. Silent information regulator 1 (SIRT1), hemeoxygenase 1 (HO-1), and nuclear factor erythroid 2 (Nrf2) mRNA expressions were detected. Mallory-stained colonic sections were examined for collagen fibres detection. Immunohistochemistry of NF-κB and p53 expressions in colonic sections were assessed. Results There acetic acid induced colitis increments in MDA, IL-1β, TNF-α, and caspase 3 levels while decreased SOD, pAkt, SIRT1, HO-1, and Nrf2 with increased collagen fibres as well as NF- κB and p53. Icosapent decreased macro& microscopic colitis scores, MDA, IL-1β, TNF-α, and caspase 3 levels while increased SOD, pAkt, SIRT1, HO-1, and Nrf2 with decreased collagen fibres as well as NF-κB and p53. The effects of icosapent 300 mg/kg were similar to mesalamine. Icosapent effects were antagonized by EX527. Conclusion Icosapent alleviated acetic acid-induced colitis via its anti-inflammatory, Antioxidant, and anti-apoptotic effects mediated in part by SIRT1 pathway activationIcosapent ethyl alleviates acetic acid-induced ulcerative colitis via modulation of SIRT1 signaling pathway in rats

Disseminated neurological symptoms caused by acute and chronic inflammation include loss or alteration of sensation (numbness, paresthesia), of motor function (typically spastic paresis or complete paralysis), visual symptom such as blurring of vision, transient blindness, disorder of conjugate eye movements, bladder, bowel dysfunction, and cognitive impairment. MS patients require enduring medical and rehabilitative care. Fatigue is one of the most common symptoms reported by patients with MS, which is reported to affect between 50% and 80% of patients.[1]The diagnosis of MS is based on representing evidence of inflammatory-demyelinating harm within the CNS that is disseminated in both time and space. Diagnosis is through a combination of the clinical history, neurologic examination, magnetic resonance imaging, and the exclusion of other diagnostic possibilities. Other so-called “para-clinical” tests, including the examination of the cerebrospinal fluid, the usage of evoked potentials, urodynamic studies of bladder function, and ocular coherence tomography, may be helpful in forming the diagnosis of MS, but are often unnecessary.[2]MS is characterized by episodes (“attacks” or “relapses”) of neurologic dysfunction. The symptoms caused by these attacks vary significantly between patients and depend on the site of neurologic involvement. Commonly, patients may complain of numbness, tingling, weakness, vision loss, gait impairment, incoordination, imbalance, and bladder dysfunction. Between these attacks, in the relapsing-remitting (RR) phase of the illness, patients are neurologically stable.[3]Many patients who begin with RRMS progress to the secondary progressive phase of the illness, in which they have marked worsening of function and increasing of neurologic disability not related to any acute attacks that may or may not occur. About 66% of RRMS patients developed SPMS at an average time of 15 years,[4] while in a British Columbia cohort study, 58% of patients with relapsing MS advanced to SPMS after an average time of 19.1 years.[5]Objectives: To evaluate central auditory functions in multiple sclerosis (MS) patients. Subjects and Methods: Twenty Egyptian MS patients involved in the study and 20 healthy controls who were matched to MS group in age, gender, and literacy. They ranged in age from 30 to 50 years with a mean age 37.6 ± 5 in the study group versus 37.3 ± 4.2 in the control group. In this study, four screening tests were conducted for the assessment of central auditory processing: Gaps In Noise test (GIN), Arabic dichotic digits test (DDT), Pitch Pattern Sequence test (PPS), and last Arabic Speech In Noise test (SPIN). Results: The study showed elevated GIN test approximate threshold and depressed total score in MS patients compared to the control group, with no significant difference between both right and left ears. There was statistical significant depressed scores in study groups at PPS, speech intelligibility in noise, and DDTs. Conclusion: The findings of the current study add more evidence to the involvement of central auditory processing abilities in patients with MS. The assessment of central auditory function is highly recommended for all MS patients as a routine examination and can be used for monitoring the effectiveness of medication and related therapies for these patients

ABSTRACT: Background: Interstitial lung diseases (ILD) are a group of diffuse parenchymal lung disorders associated with cough, dyspnea, hypoxemia and restrictive pulmonary function. Vertigo and dizziness are early symptoms of cerebral hypoxia Aim: The aim of this study was to assess the effect of hypoxia and chronic cough in patients with ILD on central and peripheral vestibular functions. Method: A cross sectional study was conducted in Audio-vestibular unit, ENT Department and Chest Department, Zagazig University Hospitals. Sixty two patients diagnosed to have ILD were included. Full VNG test battery was done. Results: There was statistical significance decrease in mean of optokinetic speed and smooth pursuit gain at high frequency (0.6) in both Rt & LT sides among cases who had moderate hypoxia compared to cases who had mild hypoxia). No difference was found between mild and moderate cases in other oculomotor tests parameters. Also, there was statistical significance increase in frequency of BPPV among cases who had moderate hypoxia compared to cases who had mild hypoxia.
