Proposal of a New Predictive Score for the Requirement of Intensive Care Unit Admission Among Ethanol-Intoxicated Patients

Abstract:
The correct selection of ethanol-intoxicated patients that require ICU admission is crucial for reducing morbidity and mortality. This study included 165 patients intoxicated with ethanol, confirmed by blood analysis. The patients were divided into ICU group = 46 and inward group = 119. We created a new predictive score based on seven parameters of APACHE II score and seven laboratory parameters that were significant in ICU-admitted patients by giving each parameter one point. The new predictive score achieved a sensitivity of 87, a specificity of 93.3, and an area under the curve of 0.967 with a cutoff >6. On the other hand, blood ethanol level (BEL) exhibited a weak correlation with both APACHE II and new predictive scores and a weak correlation with the parameters of the new predictive score. The new predictive score included 14 parameters. This score can be used in predicting whether patients need ICU admission during assessment for ethanol intoxication in emergency rooms, independent of BEL analysis. It could potentially reduce morbidity and mortality
