Diagnostic validity and reliability of BT- RADS in the management of recurrent high-grade glioma

ABSTRACTBackgroundandpurpose:BT-RADSisanewframeworksystemforreportingthetreatmentresponseofbraintumors.TheaimofthestudywastoassessthediagnosticperformanceandreliabilityoftheBT-RADSinpre-dictingtherecurrenceofhigh-gradeglioma(HGG).MaterialsandMethods:Thisprospectivesingle-centerstudyrecruited81caseswithpreviouslyoperatedandpathologicallyprovenHGG.Thepatientsunderwentbaselineandfollow-upcontrast-enhancedMRI(CE-MRI).Twoneuro-radiologistswithtenyears-experienceinneuroimagingindependentlyanalyzedandinter-pretedtheMRIimagesandassignedaBT-RADScategoryforeachcase.ToassessthediagnosticaccuracyoftheBT-RADSfordetectingrecurrentHGG,thereferencestandardwasthehistopathologyforBT-RADScate-gories3and4,whileneurologicalclinicalexaminationandclinicalfollowupwereusedasareferenceforBT-RADScategories1and2.Theinter-readeragreementwasassessedusingtheCohen’sKappatest.Results:Thestudyincluded81casesofHGG,ofwhich42wererecurrentand39werenon-recurrentHGGcasesbasedonthereferencetest.BT-RADS3BwasthebestcutoffforpredictingrecurrentHGGwithasensi-tivityof90.5%to92.9%,specificityof76.9%to84.6%,andaccuracyof83.9%to88.9%,basedonbothreaders.TheBT-RADSshowedasubstantialinter-readeragreementwithaKof0.710(P=0.001).Conclusions:TheBT-RADSisavalidandreliableframeworkforpredictingrecurrentHGG.Moreover,BT-RADScanhelpneuro-oncologistsmakeclinicaldecisionsthatcanpotentiallyimprovethepatient’soutcome.
