Nutritional Status and Depressive Symptoms among Elderly Cancer Patients Receiving Chemotherapy at Zagazig University Hospitals

Abstract:
Background: Malnutrition and depression are among the most important problems affecting the elderly treated with chemotherapy, as they negatively affect the quality of life, treatment outcomes, chemotherapy tolerance, and causing short survival. The aim of this study was to assess nutritional status and depressive symptoms among elderly cancer patients receiving chemotherapy at zagazig university hospitals. Design: A descriptive design was utilized. Setting: The study was conducted at the oncology day clinic and oncology department at Zagazig University Hospitals. Sample: A purposive sample of 194 elderly with cancer receiving chemotherapy. Tools: Three tools were used; I) A structured interview questionnaire to collect data on demographic characteristics, medical history, and dietary profile. II) Mini Nutritional Assessment (MNA). III) Geriatric Depression Scale short form. The results revealed that 33% of the elderly were malnourished and 51.5% were at risk of malnutrition. Depressive symptoms were marked in 68.6% of the elderly, of which 37.1% had mild levels. Both malnutrition and depression demonstrate statistically significant relations with illiteracy, comorbidities, insufficient income, advanced cancer stage, receiving four or more chemotherapy cycles, and receiving other cancer treatment. A statistical negative correlation was found between nutritional status and depressive symptoms. Conclusion: Malnutrition and depressive symptoms were representing high prevalence among elderly cancer patients receiving chemotherapy. Moreover the elderly with more severe malnutrition had a higher level of depressive symptoms. Recommendations: Pre-treatment and ongoing screening programs for early detection of malnutrition and depression. Conducting nutritional and psycho-educational programs to improve nutritional status and depressive symptoms among elderly patients receiving chemotherapy are also recommended.
