Effect of Nursing Intervention on Uremic Pruritus and Related Quality of Life of Elderly Patients on Hemodialysis

Abstract:
Background: Uremic pruritus is a major public health problem and is a common and troublesome complication experienced by chronic kidney disease patients undergoing hemodialysis. Further, the oldest patients are the fastest growing subgroup of the dialysis population in developed countries. Older hemodialysis patients with uremic pruritus are suffering from increased morbidity and mortality and decreased quality of life. Aim of the study: The present study aimed to evaluate the effect of nursing intervention on uremic pruritus and related quality of life of elderly patients undergoing hemodialysis. Research design: A quasi-experimental pretest-and-posttest design was utilized. Setting: The study was conducted at the hemodialysis unit at the internal medicine department of Zagazig University Hospital and the hemodialysis unit at Al Ahrar hospital affiliated to the Ministry of Health. Subject: This study included 70 elderly patients. Tools: Three tools were used. Tool I: Structured Interview Questionnaire which consisted of part 1; demographic characteristics and part 2; health history and clinical data of elderly patients. Tool II: The 5-D itch scale. Tool III: Dermatology Life Quality Index (DLQI). Results: The results revealed a post intervention highly statistically significant difference in the total mean score of pruritus among studied elderly patients undergoing hemodialysis (P=.000). Post the nursing intervention, the dermatology life quality among studied elderly patients on hemodialysis had greatly improved at post intervention phase. Conclusion: The nursing intervention is effective in improving uremic pruritus and related quality of life of elderly patients on hemodialysis, and can be considered a safe and effective approach to improve elderly's quality of life. Recommendations: It is recommended to replicate this study using a randomized clinical trial design in order to confirm the findings and to provide a higher level of evidence of its findings. Hemodialysis units must involve nursing intervention for improving uremic pruritus of hemodialysis patients.
