SAFETY AND EFFICACY OF THE PTERIONAL CRANIOTOMY FOR HUGE OLFACTORY GROOVE MENINGIOMAS

Abstract:
Objective: We focused on the feasibility and reliability of microsurgical resection of challenging huge olfactory groove meningioma projecting backwards towards the suprasellar compartment using the simple and safe pterional approach. Methods: This is a retrospective analysis of 12 patients with huge olfactory groove meningiomas who were operated on using the pterional approach between 2013 and 2018, at Neurosurgery Department, Zagazig University Hospitals, Egypt. We reviewed the preoperative clinical and radiological data, the intraoperative findings, as well as the postoperative data focusing mainly on the extent of tumor removal, approach-related complications, early postoperative outcome, and tumor recurrence. Results: Early exposure and dissection of arteries of the anterior circulation, and optic nerves was feasible in all cases. This allowed for complete tumor removal (Simpson grades I, II or III) in all but one patient, in whom subtotal removal (Simpson grade IV) was achieved because the tumor was encasing the right optic nerve and anterior cerebral artery. Early postoperative improvement of visual disturbance was achieved in 2/6 patients. Behavioral changes markedly improved in 5/7 patients. We had no major neurological or approach-related morbidity in the early postoperative period. No tumor recurrences were observed during follow-up. Conclusion: The pterional craniotomy is a simple, safe, and successful method in the management of huge olfactory groove meningioma, achieving excellent outcome comparable, if not superior, to the more commonly used frontal approaches.
