Abstract:
Objective: To evaluate the efficacy of the autologous platelet-rich plasma (PRP) in the treatment of chronic diabetic foot ulcers. Summary Background Data: Diabetic foot ulcer is a major complication of diabetes mellitus with increased financial burden, morbidity and mortality. Over the recent years, considerable progress in the techniques that promote wound healing has been achieved, of which autologous PRP is gaining popularity. Platelets start the wound healing process through the release of locally active growth factors. PRP is thought to stimulate wound healing by releasing essential growth factors for healing. Methods: We conduct a prospective randomized controlled study of 56 patients of chronic diabetic foot ulcers who were equally subdivided into two groups. The first group of patients was managed by activated PRP injection and gel on the surface of the ulcer. The second group was managed by conventional dressing using sterile saline and wound covering. Results: Both methods of treatment improved the healing in both groups, but there was a statistically significant increase in the healing rate among the PRP group {(25 (89.28%) vs. 16 (57.14%)} (p= 0.007). Also, the duration of healing was shorter in the PRP group (p= 0.026). Conclusions: Conventional wound care in the form of wound debridement and removal of callosities around the wound can improve wound healing. The use of autologous platelet-rich plasma gives a higher rate of wound healing than the conventional method.
