Early and Late Management of Non-penetrating Traumatic Thoracic Tracheobronchial Injuries

Abstract:
Objective: Evaluation of early and late management of tracheo-bronchial injuries (TBIs). Methods: The reports of 32 patients from February 2008 to November 2011 were reviewed. The 27 males and 5 females had a mean age of 26.3 years (range: 7–50).Twenty six patients (81.25%) had acute presentation and six patients (18.75%) had chronic presentation. Prominent symptoms in acute patients were dyspnea, subcutaneous emphysema. Associated injuries were seen in 10 acute patients. The most frequent symptoms in chronic patients were dyspnea, suppurative cough, intermittent fever, and consolidation collapse. Bronchoscopic detection of TBIs was the main indication for surgery. Eight acute patients were managed conservatively while the other 18 acute patients and all chronic patients underwent thoracotomy. Results: Twenty four patients (75%) underwent thoracotomy. In acute patients, repair was done in 15 patients (83.3%) and resection was done in 3 patients (16.7%). In chronic patients, repair was done in one patient (16.7%) and resection was done in 5 patients (83.3%). Morbidity was observed in 5 patients (15.6%); 4 showed stenotic suture line and responded to bronchoscopic excision of granulation tissue, and one showed pneumonia and responded to antibiotics. Three patients died (9.4%).There were associated injuries in 100% of patients that died and in 24% of those that survived. Conclusion: In civilian life, TBIs occur relatively rarely. Bronchoscopy should be performed in all suspected patients. 190Conservative management must be applied in the indicated cases. Operative management can be performed with acceptable mortality. Associated injury is an important mortality factor.
