Objective of the study: The purpose of this research is to review the technique of needle tenotomy of the Achilles tendon as a step of Ponseti method and to discuss its results. Background: Achilles tentomy is the last step of treatment of talipes equinovarus tenotomy with Ponseti technique. It may be needed in up to 85% of cases, traditionally it was done using surgical blade but later modification to be done with needle has been evolved. This technique has many advantages with minimal morbidity. Patients and methods: 35feet in 24 patients of idiopathic talipes equines had serial manipulations with Ponseti technique. Percutaneous needle Achilles tenotomy as an OPD procedure Results: all cases had immediate clinical improvement after the procedure without any recurrence during the follow up period. Conclusion: Needle tentotomy can replace traditional blade due to a lot of advantages and minimal drawbacks
