Abstract: background: The relative rarity of synchronous or metachronus para-aortic lymph node (PALN) in colorectal carcinoma (CRC) patients lead to limited number of studies on patients management and up-till now there are no established treatment guidelines. Aim of the work: was to assess prognostic, predictive roles and long term outcomes of different management strategies for isolated metachronus or synchronus PALN metastases in patients with CRC to establish its management strategy. Patients and methods: we included 35 CRC patients with solitary metachronus PALN recurrence and 25 patients with solitary synchronous PALN metastases in CRC patients who underwent curative R0 resection. PALD was performed in 15 cases of the first group and in 10 cases of the second group all remaining patients in both groups underwent chemoradiotherapy (CRT) without further surgical intervention. During the study period of about 5 years we compared between patients underwent surgical intervention and those who underwent CRC. Results: overall survival rate and recurrence free survival rate were significantly longer in patients who underwent PALN resection (p=0.049 and 0.036 respectively). Conclusions: We showed that resection of isolated PLN recurrence in CRC patients and PALND in R0 CRC patients was associated with favorable outcome and better patients’ survival.
