Background: The age for maternal reproduction has been estimated to be between 15-49 years, but the ages between 20-35 years has been considered as the safest age group for bearing a child. This implies that pregnancy or childbirth below or above this age range can have unpleasant influences on either the mother, the pregnancy, delivery or the child. So the aims of the present study were to compare nulliparous pregnancy outcome in adolescence and advanced maternal age versus safe reproductive age and set nursing guidelines to orient nurses about caring of nulliparous adolescents and advanced maternal age. Design: A descriptive research design was used. Setting: The study was conducted at the labor ward of labor and childhood hospital at zagazig university hospitals, Egypt. Subjects: 195 nulliparous women were included and divided into three groups, aged less than 20 years (adolescents group, 65); with the mothers aged from 20 to 35 (safe reproductive age group, 65) aged more than35 (advanced maternal age group, 65).Tools of data collection: Three main tools, structured interview questionnaire, labor assessment record, and neonatal assessment record. Results: The nulliparous adolescents had significant higher percentages of previous abortion, threatened abortion in the current pregnancy and anemia. In addition to significant higher percentages of labor complications as post term delivery, prolonged labor more than 12 hours, first and second degree perineal tears. Moreover, fetal distress and labor progress failure which significantly indicated CS than the other age groups. While the nulliparous advanced maternal age were more likely to have severe preeclampsia, fetal distress and conception by assisted reproductive technology, which significantly indicated caesarean section. Also second degree perineal tears significantly affected advanced maternal age group than the other age groups. In conclusion, maternal age between 20-35 years was the safest reproductive age. Nulliparous adolescents and advanced maternal age experienced many pregnancy and labor complications, while there was no statistically significant difference among the three studied groups regarding neonatal outcome. Recommendations: Nurses should counsel women before being pregnant about the complications that may face them and emphasis on the importance of the early and good enough prenatal care in case of adolescents and advanced maternal age.
