Effect of Different Ischemia Times on Recovery of Kidney Functions and Morphology after Solitary and Paired Partial Nephrectomy

Abstract:
Abstract—Several studies reported that the ischemic kidney injury was associated with the chronic and acute renal injury. The target of current study was to compare the different ischemia time’s effect on the recovery of the kidney functions and morphology as well as to compare the tolerability of the paired partial nephrectomy and solitary kidneys to renal ischemia. Methods: Two main groups (N=18) were used; Group (1); to create solitary kidney with open right nephrectomy, Group (2); the paired kidney model. The two groups were subdivided into three subgroups according to the time of the renal ischemia as following: 30, 60 and 90 min. Differences were evaluated biochemically using glomerular filtration rate (GFR) and morphologically by HE and MT. Immunohistochemical evaluation of apoptosis (by CD95) and renal cell proliferation and regeneration (by PCNA) were also used. Results: It was found that the higher the renal ischemia duration, the higher the impairment in both paired and solitary kidney. The serum creatinine increase and GFR reduction of ischemic group were correlated with increasing the duration of ischemia length. In consistence, histopathological examination exposed tubular necrosis and tubulointerstitial destruction in ischemic kidneys which is proportional with the duration of ischemia. Apoptosis and necrosis of renal tubules with dilatation and atrophy of renal tubules were the main complication noted in ischemic kidney. Furthermore, the expression of both CD95 and PCNA were significantly increased in kidney tissues with increasing the ischemia time. In conclusion, as ischemia generate many complications in kidney function and morphology, solitary kidney was noted to be more resistant to these complications than paired kidney.
