Short-term results of treatment of Legg-Calve´-Perthes disease by femoral varus derotation osteotomy

Abstract:
Abstract Background: The goal of treatment of Legg-Calve´-Perthes disease has been to prevent femoral head deformation by containment of the head within the acetabulum, this is can be achieved by many methods. The purpose of this study is to evaluate the Short-term results of treatment of Legg-Calve´-Perthes disease by femoral varus derotation osteotomy. Patients and methods: Thirteen patients with unilateral Legg–Calve´–Perthes disease were treated using proximal femoral varus derotation osteotomy. The mean age of the patients was 7.3 years (range: 6-8.6 years). The average follow-up was 32.6 months (range: 28–36 months). Based on the Catterall and Herring classifications, the 13 patients were classified as follows: three patients were Catterall group III (Herring group B) 23.1%, two patients were Catterall group III (Herring group BC)15.4%, and the other eight patients were Catterall group IV (Herring group C) 61.5%. Results: Clinical and radiologic results were evaluated. Clinically, no hip pain was encountered in all patients, the hip motion was good in 11 hips (84.6%) and fair in 2 hips(15.4%), gait was good in 4 hips (30.7%) and fair in 9 hips (69.3%), shortening was 0.5cm (good) in 10 patient (76.9%) in whom lateral opening wedge resection was used as compared with 1 cm (fair)in 3 hips (23.1%)in whom medial closing wedge resection was used. Radiologically, post-operative Wiberg’s center edge angle was good for all hips, and the neck-shaft angle didn’t fall below 105 degrees in any of the patients. Conclusion: According to the results of the present study, proximal femoral varus derotation osteotomy gives good results in early presented young children with no evidence of femoral head flattening, deformity, or subluxation, especially those with good containment in the abduction.
