Pronator quadratus pedicled bone graft versus volar peg graft for nonunited scaphoid fractures

Abstract:
Abstract Background: Scaphoid nonunion often presents a therapeutic challenge. The goal of this study was to compare the clinical outcome of the pronator quadratus pedicled bone grafts to that of the volar peg bone graft in scaphoid nonunion. Patients &Methods: During the period from, June 2012 to September 2017, we enrolled in our study 41 consecutive patients who suffered scaphoid nonunion. We analyzed the results of 39 patients who completed the 12 months follow-up period. Patients were treated surgically with either pronator quadratus pedicled bone graft (group A) or volar peg graft (group B). Radiographic evaluation was considered the primary (objective) outcomes data which assessed fracture union, time to consolidation, and carpal bones geometry. Secondary (subjective) outcomes data were assessed according to Mayo Wrist Scoring System. Results: At the final follow-up interview, union was achieved in 17/19 (89.5%) of patients in group A including 3/5 (60%) scaphoid nonunions with AVN, the mean postoperative Mayo scoring system was 84.2±27.8. Union was achieved in 16/20 (80%) of the patients with volar peg graft (group B) including 1/5 (20%) scaphoid nonunions with AVN achieved union; the mean postoperative Mayo scoring system was 76.9±39.9. Conclusions: The results of this study showed statistically significant higher union rate and faster speed of radiological union in group (A) than group (B). However, the other evaluation parameters showed comparable results between the two groups. We also concluded that the pronator quadratus pedicled bone graft might be superior to a volar peg graft in treating scaphoid nonunion with proximal pole necrosis.
