Comparative study between closed and open reduction internal fixation of acutely displaced unstable lateral humeral condyle fracture in children

Abstract:
Background: Lateral humeral condyle fracture in children is considered the second most common type of fracture around elbow in children after supracondylar fracture. Conservative treatment of these types of fracture usually fails. Open reduction and internal fixation of these types of fracture was the treatment of choice as regard anatomical reduction and stable internal fixation. Complication of open reduction was noticed ranging from mild complications e.g scar of the skin incision to severe e.g avascular necrosis of the lateral condyle. Closed reduction and percutaneous fixation of the fracture may give the same results of open reduction as regards reduction and healing of the fracture but without complications of open reduction. Design: prospective study. Level III Methods: this prospective intervential study was conducted on 60 patients had lateral humeral condyle fracture grouped equally into two groups first one managed by closed reduction and internal fixation and second one managed by open reduction and internal fixation. Results: the two groups were compared after the operation and one year follow up there was no difference between the two groups as regards fracture healing and complications except complications of wound healing and skin scar after open reduction. Conclusions: closed reduction and percutaneous pinning of lateral humeral condyle fractures amenable to this type of treatment gives the same results of bone healing without complication of skin incision healing as in open reduction.
