Uterine Artery Doppler indices: Pulsatility Index and Resistance Index as a predictive tool for the incidence of Heavy Menstrual Bleeding related to Copper intrauterine contraceptive device (IUCD)

Abstract:
Objective 3 To assess an initial uterine artery doppler indices predictability value; Pulsatility Index (PI) 4 and Resistance Index ( RI) in the prediction of heavy menstrual bleeding before and after 5 Copper IUCD insertion. 6 Methods 7 Our current prospective clinical study was carried on 100 women intending to use a copper 8 IUCD (Cu T-380A) and met the requirement of inclusion criteria. Uterine artery doppler PI 9 and RI indices had calculated before IUCD insertion, at 3 and 6 months after insertion. Based 10 on the occurrence or absence of menorrhagia, all women were classified into two groups: non11 bleeding (n = 52) and extreme menstrual bleeding (n = 48). To determine the predictive value 12 of uterine artery PI and RI in prevision of menorrhagia relevant to IUCD, the receiver 13 operating curve analysis (ROC) was used. 14 Results 15 The PI and RI indexes display a high significant difference between the IUCD groups three16 and six-months following insertion (p-value < 0.001). Uterine artery PI ≤ 2.02 (sensitivity 95.8 17 %, specificity 100%, and area below the curve (AUC) of 0.97 at p-value < 0.001) and RI ≤ 18 0.83 (sensitivity 93.8 % and specificity 100%, with AUC of 0.949 at p-value < 0.001) is 19 correlated with a significant menstrual bleeding following insertion of IUCD. 20 Conclusion 21 The presented results endorse our assumption that the initial studies of uterine artery Doppler 22 can help to predict of a heavy-menstrual bleeding associated with IUCD and therefore should 23 be conducted in advice women pursuing reversible contraception with long-acting behavior. 24 25 Key words: Copper IUCD, Transvaginal Doppler ultrasound, pulsatility index and resistance
