Abstract: |
Background: ciprofloxacin is an important antimicrobial agent's belong to fluoroquinolones groups and widely used in treatment of infectious diseases. The safety and hazards of this drug on pregnancy had been discussed among the little studies.
Objective: The present study was designed to evaluate the possible structural changes in kidney, liver, lung and placenta of both pregnant albino rats and their fetuses induced orally by two selected therapeutic doses of Ciprofloxacin during gestation to adjust and control use of this drug according to its hazards and benefits during pregnancy.
Material and Methods: after mating, thirty pregnant female rats were divided randomly into three equal groups (10 rats for each). Group (I): control group: mothers received distilled water and balanced diet, Group (II): mothers received single dose (SD) of Ciprofloxacin (57mg/kg /day dissolved in Distilled water), Group (III): mothers received double dose (DD) of Ciprofloxacin (114 mg/kg dissolved in distilled water) by gastric intubation from 8th day of pregnancy to 20th day of pregnancy. At day 21 of pregnancy, from each group, the mothers were anesthetized, their abdominal and pelvic cavities were opened, the fetuses were delivered and placentae were rapidly removed. The delivered fetuses were divided into three subgroups naming according to the group in which they were delivered. At the end of experiment, the kidneys, liver and lungs of both mothers and their fetuses were rapidly removed and dissected out carefully. These specimens in addition to placentas were prepared and examined by light microscopy, Immunohistochemical, morphometric studies and statistical analysis.
The Results: by light microscopy, it was found that in the SD of ciprofloxacin treated groups, moderate degenerative lesions in all 4 studied tissues, however, was massive in the DD treated groups of both adult pregnant albino rats and their fetuses. The immunohistochemical (IHC) expression of Inducible Nitric Oxide Synthase Enzyme (iNOS) in all prepared sections of different tissues showed minimal positive immune reaction in the control group while was moderately positive in the SD treated groups and strongly positive in the DD treated groups of both adult pregnant albino rats and their fetuses. Morphometry and Statistical analysis showed high significant difference (P< 0.001) in the mean values of area percentages (area %) of iNOS in all studied tissues among different groups and subgroups of the present study.
Conclusion: The administration of either single or double therapeutic doses of ciprofloxacin during the period of pregnancy, especially during organogenesis, was harmful and exerted histopathological toxicity in different organs of both adult pregnant albino rats and their fetuses. Therefore, it is recommended to become aware about the effect of ciprofloxacin toxicity on pregnancy to limit or avoid administration of this drug during it.
