Abstract:
Objective: To reportthesensitivityandspecificity ofneuralprecursorcell-expresseddevelopmentallydown-regulatedprotein9 (NEDD9) proteinhighexpressioninpredictingBCGresponseanditseffectonrecurrenceorprogressionfreesurvivals. Patients andmethods: Between May2014andApril2017,aprospectivecohortstudywasconductedon105patientswithintermediate or high-risknonmuscleinvasivebladdercancer.Immunohistochemicalstainingwithmousemonoclonalanti-NEDD9antibodywasdone. Examination oftheslideswasdonetodetectNEDD9cytoplasmicexpression.IntravesicalinductionbacillusCalmette-Guerin(BCG) instillation therapywasstartedforallpatients2to4weeksafterendoscopywithonceweeklyinstillationfor6weeks.Intravesical maintenance BCGinstillationwasthencontinuedevery3monthsforatleast1year.Followupwithcystoscopyevery3monthsfor2years and every6monthsafterthat. Results: The meanage ± SD ofthepatientswas57 ± 4.5 years.Themedianfollow-upperiodwas25(12–36) months.NEDD9protein was highlyexpressedin61(58.1%)patients,whereaslowexpressionwasin44(41.9%)patients.Sensitivityandspecificity ofNEDD9high expression inpredictingrecurrencewere83.6%and64.2%,respectively.Itssensitivityandspecificity inpredictingprogressiontomuscle invasion ordistantmetastaseswere91.3%and51.2%,respectively.Itssensitivityandspecificity inpredictingcompleteresponsetoBCG therapy were84.1%and80.9%.Kaplan-Meiertestshowedhighlysignificant difference(P o 0.0001) insurvivalwithoutrecurrenceor progression betweenpatientsshowinghighexpressionandthoseshowinglowexpression. Conclusion: NEDD9 proteintissuemarkercouldbeusedasapredictivemarkerforBCGresponseinnonmuscleinvasivebladdercancer with reasonablesensitivityandspecificity.
