Molecular characterization of oxacillinases among nosocomial Acinetobacter baumannii isolated from a Saudi hospital

Abstract:
Background:Acinetobacterbaumanniihassuccessfullybecomeasignificantnoso-comialpathogenbecauseofitsremarkableabilitytoacquireantibioticresistanceandtosurviveinnosocomialenvironments.Thisstudyaimedtodeterminethedrugsusceptibilitypatternsandthedistributionoffoursubgroupsofcarbapenem-hydrolyzingclassD-lactamases(OXA-carbapenemases),aswellastheirinsertionsequences(ISAba1),amongA.baumanniinosocomialisolatesfromaSauditertiarycarehospital.Methods:Atotalof108non-duplicateA.baumanniiisolateswereidentified,andtheirsusceptibilitiestodifferentantibioticsweredeterminedusingthebreakpointmethod.Isolateswerethensubjectedtomultiplex-PCRtargetingblaOXAgenes. Results:Morethan75%oftheisolatesshowedresistancetodifferentantibiotics.Theratesofsusceptibilitytocolistin,meropenem,imipenemandtrimethoprim—sulfamethoxazolewere95.6,50,48.1and34.3%,respectively.AllisolatespossessedablaOXA-51-likegene.Ofthe56carbapenem-resistantisolates,48isolates(85.7%)car-riedblaOXA-23-like,3isolates(5.4%)carriedblaOXA-40-likeandtwoisolates(3.6%)hadblaOXA-58-likegenes.TheISAba1elementwasfoundupstreamoftheblaOXA-23andblaOXA-24genesin40(71.4%)and3(5.4%)isolates,respectively,whileitwasdetectedupstreamofblaOXA-51inonlyone(1.8%)isolate.Conclusion:Ourfindingsfurtherillustratethechallengeofincreasingcarbapenem-resistanceinA.baumanniiisolatesinSaudiArabia.ThehighdistributionofclassDcarbapenemase-encodinggenes,mainlyISAba1/OXA-23andISAba1/OXA-24car-bapenemases,isworrisomeandpresentsanemergingthreatinourhospital.Localmolecularsurveillanceisessentialtohelpcontrolcarbapenem-resistantA.bauman-niinosocomialinfectionsandtopreventDNAexchangeamongendemicnosocomialpathogens
