Knee Sepsis Following ACL Reconstruction – A Modified Approach for Management

Abstract:
Abstract Background: Knee sepsis after ACL reconstruction is disastrous rare condition. This condition should be managed as an emergency. Repeated lavage and arthroscopic debridement with antibiotics is the main line of treatment recommended by many authors. Repeated suction irrigation following single arthroscopic debridement is another option for treatment. Patients and Methods: 17 cases from 750 were infected after anatomical ACL reconstruction using hamstring autograft. They were managed by single arthroscopic debridement then repeated suction irrigation. Clinical and radiological monitoring of the patient during irrigation period was done for all cases. Results: Rapid improvement of clinical and laboratory parameters during the period of irrigation recorded. Recurrent infection found only in one case. The mean modified Lysholm score at the final follow up was 79.7. The graft was discarded in one case while retained in 16 cases. Conclusion: The technique of single arthroscopic debridement and repeated suction lavage is successful compared to repeated arthroscopic debridement with lower cost. It is more tolerated by patients without repeated surgical trauma.
