Abstract:
Within the past century, marked improvement in replantation/ revascularization surgery has occurred. Hand surgeons must be skilled at microvascular anastomosis and be able to achieve a 90% patency rate in a 1-mm-diameter vessel in laboratory animals. Viability of the replanted limb is no longer the sole determinant of success; functional recovery, preoperative and postoperative risks, and duration of treatment are vital factors in making the decision to replant. A total number of 111 patients with 158 severed parts of the digits, hands and forearms were operated upon with clear inclusion and exclusion criteria. Replantation/revascularization success rate approaching 90% was achieved in this series. Also functional recovery, both motor and sensory, was recorded and found to be very satisfactory. Replantation/revascularization surgery is considered now to be the gold standard of care for amputations of the digits, hands and forearm. Reasonable functional recovery could be obtained in a good number of patients specially when the patient is selected carefully and managed in a standard way
