Abstract:
Background: Hepatocellular carcinoma (HCC) is the most common primary hepatic malignancy worldwide, with an increasing incidence in western countries.It is the most common cause of death in patients with cirrhosis and accounts for 5.6% of all cancers. It is the fifth most common cancer in the world and the third most common cause of cancer death. It is accounting for over half a million deaths per year. Many risk factors seem to predispose HCC, which either present individually or collectively depending on the environmental situations. In a majority of cases, HCC occurs in the setting of cirrhosis with underlying chronic viral hepatitis (B or C) or alcoholism and more recently with non-alcoholic steatohepatitis. In western countries, HCC is associated in up to 80% of cases with chronic liver damage. The diagnosis is often made at an advanced tumour stage, precluding curative options such as surgical resection, transplantation or local ablation therapy.Objectives: The aim of this study was to see the pattern of clinical presentation, identify the possible risk factors and the clinical outcome among HCC patients presenting at our hospitalPatients and methods: In our study there were 120 HCC cases diagnosed over one year period giving an overall proportion of 11.2%. HCC was diagnosed from total of 1069 Egyptian patients with CLDs were registered.Results: Patients who were diagnosed, 86.7% were males and 13.3% females and most of them diagnosed mainly among males between( 30 – 50) years. HCV,HBV infection and Schistosomiasis were more prevalent among HCC positive cases than HCC negative cases. D.M., Obesity, Smoking also, were more prevalent among HCC positive cases than HCC negative cases. Diagnosis of HCC was by pelviabdominal U/S and AFP level. HCC has a poor prognosis as most of the patients died within 6 months.Conclusion: Preventive measures is very important in our country to stop this circle and decrease prevalence of HCV in our country.
