Evaluation of Quality standards in haemodialysis unit in Zagazig university hospitals

Abstract:
End Stage Renal disease [ESRD] represents one of the major health problems whose management needs adherence to quality standards. It is imperative that quality assessment and improvement is the way of the future in the health services in general and haemodialysis in particular.Aim of the work: Improve quality standard in hemodialysis unit in Zagazig university hospital and improve level of care of patient with ESRD through assessment of quality standards and effect of a suitable intervention on it.Subjects and methods: An intervention study was carried out in haemodialysis unit at Zagazig university hospital. 1st phase was assessment of quality standards of haemodialysis unit. 2nd phase was intervention for the lowest mean percentage of compliance of standards which was infection prevention and control. All residential doctors, nurses and sanitary workers working in the unit were included in the study. All staff member were interviewed to assess their knowledge and attitude regarding infection control practice. The performance of nurses responsible for dialysis and sanitary workers only was observed.Results: The lowest mean percentage of compliance to standard was for infection prevention and control. The lowest level of knowledge in pretest: was detected in using gloves (0.0%) which was improved to (92.2%) after intervention with statistical significant (p value=0.00), importance of washing hands before replacement of torn gloves with a new pairs (58.8%) improved to (82.4%) with statistical significant (p value = 0.0). The least level of attitude in pretest was towards effectiveness of wearing personnel protective equipments in preventing hospital infection although high effectiveness represents (80.4%) improved in posttest to (100.0%) with no statistical significant difference. The worst of practice: was in practice of wearing gloves before insertion of dialysis catheter (0.0%) which was improved to (29.4%) after intervention with statistical significance (p value=0.00), Practice of hand hygiene before insert dialysis catheter was (32.4%) and improved to (61.8%) after intervention with statistical significance (p value= 0.021).Conclusions: quality standards in haemodialysis unit are not completely fulfilled and intervention can improve this situation. Knowledge and practice of health care staff in haemodialysis unit about infection control procedures can be improved significally by training and health education.
