Laparoscopic versus open surgery for ventral hernia repair

Abstract:
Objective :To assess the laparoscopic repair of ventral hernia in comparison with open mesh repair. Patients and methods: a total of 60 patients with ventral hernia who underwent surgical hernia repair at were divided into two groups; group I (open repair) and group II (laparoscopic repair). Inclusion criteria: Uncomplicated primary ventral, incisional and recurrent hernias, including irreducible hernia in adult population. Exclusion criteria: Patients with obstructed or strangulated, local and systemic infection and uncontrolled medical diseases (renal failure, etc.). Open repair: Subcutaneous flap was raised about 5 cm around the defect. Reduction of the hernia content with closure of the defect, primary by 00 polypropylene, was done. Onlay mesh, Polypropylene mesh of a suitable size, was fixed over the sheath with 2-0 polypropylene suture. Laparoscopic repair: creation of Pneumoperitonium with placement of three ports, one 10-mm for the telescope and two 5-mm trocars. Omental and bowel adhesions were taken down. Polypropylene mesh of a suitable size was used and fixed to anterior abdominal wall using transfascial. Statistical analysis: Data were coded, entered and analysed using SPSS version 15.Results: laparoscopic repair showed results better than that of open repair as regard postoperative pain, operative time, hospital stay and most of complications. Recurrence rate was higher in laparoscopic repair, two cases for laparoscopic versus one for open repair. Conclusion: Laparoscopic repair of ventral hernia is a promising solution for hernia repair which needs increasing its learning curve for optimum results.
