Abstract:
in intubated patient receiving mechanical ventilation, the endotracheal tube, ventilator circuit and other system values also add to the work of breathing and this is called the imposed work. Automatic tube compensation has been developed to overcome the imposed work of breathing due to artificial airways. It potentially simulates spontaneous breathing without endotracheal tube. So, it has been designated as electronic extubation.Objectives: The aim of the work is to assess extubation outcome after spontaneous breathing trial with ATC versus other traditional methods like PSV and CPAP and to follow the patients after extubation.Patients and methods: The previous study was conducted in surgical ICUs of Zagazig University Hospitals on one hundred and fifty patients divided into 3 groups according to the mode of weaning. Group I is ATC (100% compensation), group II PSV receiving PS 7 cmH2O and group III is CPAP group receiving 5 cmH2O CPAP. The patients were monitored for signs of good or poor tolerance and extubation or reventilation was done accordingly. Extubated patients were followed up until home discharge or death.Results: The results of these thesis can be summarized into that ATC is a safe mode for weaning from MV. ATC hastens weaning, but it does not affect mortality or length of hospital stay.Conclusion: We can conclude that ATC is an addition to the modes of weaning from MV and it is a safe method to enhance extubation.
