Abstract:
ectopia lentis is a lenticular dystopia which may be congenital or acquired in origin; it may be traumatic, degenerative or spontaneous. According to the amount of zonular support, ectopia lentis can be classified into subluxation, when the lens partially remains in the pupillary area at the posterior chamber, or dislocation, when it can migrate to the anterior chamber or vitreous cavity as a result of complete rupture of zonule fibres.Subluxation of the catarctous lens has various surgical management options. Removal of the entire lens, along with its capsule followed by IOL implantation in the AC or fixation of the IOL to the iris or sclera is the widely practiced surgical modality. Aphakia may also be managed by aphakic spectacles or contact lenses. In mild to moderate zonular dialysis, the capsular bag may be preserved and used for IOL implantation by using CTR. In cases of severe zonular dialysis, the capsular bag can still be used for IOL fixation using a Cionni ring or Ahmed segments and suturing the rings to the scleral wall.With recent advances in equipment and instrumentation, the surgeon is able to perform relatively safe phacoemulsification in the presence of compromised zonules.The advent of capsular support devices-capsule tension rings, modified capsule tension rings and iris/capsule support hooks- has given the cataract surgeons an improved intraoperative capsular support for safer cataract removal and preservation of the capsule for in the bag intraocular lens implantation.The aim of our work is to assess the safety and efficacy of phacoemulsification and implantation of foldable intra ocular lens in patients with subluxated cataract.
