Updates in surgical management of liver and lung hydatid cysts

Abstract:
The aim of this study is to discuss different ways of management ofhydatid cysts of the liver and lung including, diagnosis, ,different surgicaltechniques, postoperative care and management of complications.Background: Hydatid disease or ecchinococcosis is azoonotic infection caused bylarval stages of cestode species of the genus ecchinoccocus(atape worm), cystic formis caused by Echinococcus granulosus, alveolar form is caused byE.multilocularis,and polycystic forms are caused by either E.vogeli or E.oligarthrus.The disease is common in the sheep-raising Mediterranean regions, Australia,portions of Africa, India, Turkey, Southern Europe, New Zealand and veryuncommon in United States. Infestation by hydatid disease in human most commonlyoccurs in the liver (55-70%) followed by the lung (18-35%); the two organs can beaffected simultaneously in about (5-13%). Other organs can be affected as heart,brain, bone and spine. Clinical presentation of hydatid disease depends on the site andsize of lesion. Diagnosis is made ultrasonically, chest X-ray, CT scan, MRI are ofgreat value in diagnosing cardiac and intracranial hydatiosis. Different serologicaltests are carried out for the diagnosis,screening and post-operative follow up forrecurrence. Surgery remains mainstay of treatment,with less invasive methods andcombinations with chemotherapy coming to the fore as techniques develop. Surgicalmanagement of a pulmonary hydatid disease include the complete evacuation of thecyst with the endocyst and the avoidance of the contamination and management ofresidual cavity. Normal lung must be preserved as much as possible. For cysts withabscesses or severe pulmonary destruction, lobectomy or even pneumonectomy isrequired. Control of the E. granulosis includes long-term measures of public healtheducation with primary health care and veterinary public health activaties,such asimprovement of slaughter hygiene and meat inspection ,dog registration andsanitation measures.Objectives: The aim of this study is to discuss different ways of management ofhydatid cysts of the liver and lung including, diagnosis, ,different surgical techniques,postoperative care and management of complications.
