Abstract:
AbstractThis study was carried out in NICU and Neonatology wards in Pediatric and Clinical Pathology Departments of Zagazig University Hospitals, during a period from Jaunary 2006 to April 2007. It included 60 preterms who were clinically classified into:Group I : 24 preterms with necrotizing enterocotitis (NEC), their weight ranged from 1 to 2.3 kg, their ages ranged from 2 to 21days,and their gestational age ranged from 29 to 33 weeksGroup II : 12 preterms with NEC & sepsis, their weight ranged from 1.2 to 1.5 kg, their age ranged from 8 to 12 days, their gestational age ranged from 31 to 32 weeks.Group III : 12 preterms with sepsis, their weight ranged from 1.2 to 2.5 Kg, their ages ranged from. 2 to 6 days,and their gestational age range from 32 to 34 weeksGroup IV : 12 apparently healthy preterm neonates, their weight ranged from 1.2 to 2.5 kg, their ages ranged from 1 to 15 days, and their gestational age ranged from 29 to 34 weeks.All preterms were subjected to complete history taking and full clinical examination and screening laboratory tests including CBC, CRP , blood gases, blood cultures , serum sodium level ,measurement of expression of CD95 on lymphocytes and CD62P on platelets using flowcytometry.We aimed to evaluate the expression of CD95, CD62P in premature infants suspected to have NEC and to find out a relation between these markers and this serious disease.As regards septic cases or NEC + sepsis, about 40% of all cases have sepsis. The commonest isolated organism was Klebsiella. Pneumoniae (33.3%) of all septic cases, then other organisms E.coli, H. influenza, Pseudomonas and Cogaulase –ve staph, all have the same frequency (16.66%) of all septic cases.Regarding mean values and ranges of laboratory parameters for all studied preterms. It showed Neutopenia, thrombocytopenia, also showed rise in CRP, rise in Band%, rise in PaCo2 , decreased pH, decreased PaO2, decreased HCo3 and Na.As regards comparison among groups (sepsis), (NEC) and (NEC + Sepsis) according to age, weight and gestational age. We found that mean age values in septic group tends to be younger (3.71.8)days than NEC group (9.37.2)days, and NEC + sepsis group (10.3 1.8)days.Also we found that mean weight values in septic group tends to be more weight (1.2-2.5)Kg than NEC (1-2.3)Kg and NEC + sepsis (1.2-1.5)Kg, and finally gestational age in septic group tends to be more age (32-34)weeks than NEC (29-33) and NEC + sepsis (31-32).
