Pregnancy Rate After Laparoscopic Surgery

After scoring laparoscopic adhesiolysis was carried out using blunt . sharp techniques or with use of electro cauterization.After chromopertubation .Adhesions encapsulating the fimbriae were incised or excised using fine scissors while the agglutinated fimbriae were dilated with atrumatic forceps introduced into the ostium closed and then withdrawn opened the procedure was repeated several times . In cases of hydrosapinx a cruciate incision by fine scissors or an electrosurgical needles started at the dimple at the site of the former closure . To maintain the eversion of the edges endocoagulator or bipolar forceps was used with a light bruching to obtain the necessary desiccatin and retraction of the serosa which everted the edges of the incised tube and flowering of the tubal opening.Follow up for one year was done to the cases to determine the pregnancy rate.The second group of patients with total number of 50 cases diagnosed as clomiphene-resistant polycystic overy syndrome underwent a laparoscopic drilling.The outcome within 1 year after laparoscopic ovarian cauterization were 68% achieved spontaneous ovulation and 52% got pregnancy where 20% without any additional treatment and 32% got pregnancy after addition of clomiphene citrate for 2 cycles in adoes of 100 mg/ day for 5 days. Normal menstrual cyclicity was observed in 62% of patients after LOD.
