Abstract:
There is increasing evidence that chronic obstructive pulmonarydisease (COPD) is a multiorgan systemic disease. Skeletal muscleweakness and wasting and impaired exercise performance have been welldescribed as frequently occurring symptoms in advanced COPD.This study was planned to assess the metabolic disturbance and itsreflexion on the muscular state in COPD.The study was carried out on 60 COPD patients and 20 healthycontrol group.The subjects were classified into 3 groups as follows:Group I: Included 30 stable COPD patients.Group II: Included 30 COPD patients with acute exacerbation.Group III: Included 20 apparently healthy subject as a control group.All subjects shared in this study were subjected to the following:(1)Thorough medical history, stressing on smoking history, steroidtreatment, frequency of exacerbation and ICU admission.(2)Full clinical examination both general and local chest examination.(3)Anthropometric measurements: among them- Mid thigh cross sectional area.- Midarm muscle circumference.(4)Plain chest X-ray (P-A) and lateral views.(5) Ventilatory pulmonary function tests.6 ) Routine laboratory investigations: a-Liver function tests. b-Kidney function tests. c- Complete blood picture.(7)24 hour urinary creatinine.(8)Creatinine height index.(9)Arterial blood gases.(10)Blood samples for measurement of:- IL-6 by chemilumenscence.- Insulin like growth factor 1 by ELISA .
