Abstract:
Behçet’s disease is the most common cause of panuveitis in Egypt. The visual potentials for this disease, the lack of organized schemes for management, the fact that it strikes more those of low socioeconomic standard and its great appetite for males in their most productive years further complicated the socioeconomic burden of this disease.The clinical course of Behçet’s disease is characterized by exacerbations and remissions, and can lead to many serious ocular complications that reducing the visual acuity. Till now there is no cure for Behçet’s disease, but, there are many trials of medications typically focus on reducing discomfort and preventing serious complications. Medications include: cholichicine, NSAIDs, alpha-chemotrypsin, plasmapheresis, corticosteroids, cyclosporine A, and immunosuppressive drugs. The mainstay for therapy in Behçet’s disease is corticosteroids.There are three available routes of administration of corticosteroids which are topical, intraocular (subconjunctival, sub-tenon, and intra-vitreal), and systemic. In ophthalmology, corticosteroids applied topically or systemically have widely been used to suppress intraocular inflammation. However, topical treatment is not sufficiently effective to suppress intraocular inflammation, and the systemic treatment with steroids leads to severe side effects such as systemic suppression of the whole immune system and Cushing’s syndrome. So, for achieving high concentrations of a drug at its site of action it is best to apply it directly into the region of required action.
