المقدمة:- أن الثدييات من ذوات الدم الحار وبالتالي فهي تحتاج أن يكون معدل درجة حرارة أجسامها ثابت .- عند تغير درجة الحرارة بصورة ملحوظة عن المعدل الطبيعي فهذا يؤثر على التمثيل الغذائي للجسم مما قد يؤدى إلى الوفاة.- وجهاز تنظيم درجة الحرارة يعمل على أن تكون درجة حرارة الجسم في الإنسان حوالى 37درجة مئوية .- تثبيط جهاز تنظيم درجة الحرارة بالأدوية التخديرية والبيئة ذات درجة الحرارة المنخفضة هما السببان الرئيسيان في أن يكون المريض درجة حرارته اقل من معدلاتها الطبيعية.- وجهاز تنظيم درجة الحرارة يعتمد على الإشارات المختلفة من جميع أنسجة الجسم .- كما أن عملية تنظيم درجة الحرارة تعتمد على ثلاث مراحل :الأولى :- الإحساس بالحرارة .الثانية :- التنظيم المركزي .الثالثة :- الاستجابة المرتدة .- كما أن جميع الأدوية التخديرية تثبط بصورة ملحوظة جهاز تنظيم درجة الحرارة أللامركزي .- وهذا التثبيط له صورة مميزة وهى أن معدل الاستجابة للتدفئة يزداد بصورة بسيطة بينما معدل لاستجابة للبرودة ينخفض بصورة ملحوظة.- أن انخفاض درجة الحرارة خلال التخدير قد يكون الأكثر شيوعا فى تغيرات درجة الحرارة فيما قبل وخلال وبعد العمليات .- انخفاض درجة الحرارة ينتج من :-1- تثبيط جهاز تنظيم درجة الحرارة .2- تعرض المريض لدرجة حرارة منخفضة فى حجرة العمليات .- انخفاض درجة الحرارة الناتج عن التخدير الكلى يتميز بـ1-انخفاض سريع في درجة الحرارة مبدئيا .2-ثبات معدل ألانخفاض3-أخيرا تثبت درجة حرارة الجسم وتبقى غير متغيرة .وكل مرحلة لها سبابها الخاص .• كما أن التنظيم اللامركزى لدرجة الحرارة يتثبط خلال التخدير الموضعي ونتيجة لذلك تنخفض درجة حرارة الجسم ويؤدى ذلك إلى الزفزفة .• وارتفاع درجة الحرارة الجسم أكثر خطورة من انخفاض درجة الحرارة وهو يعنى أن درجة حرارة الجسم قد تعدت بالزيادة المعدل الطبيعي وهى مشكلة تستوجب التدخل السريع للطبيب .- سيتم مناقشة بعض النقاط فى هذا البحث:-1- التنظيم الطبيعي لدرجة حرارة الجسم.2- تنظيم درجة الحرارة خلال التخدير الكلى.3- متابعة درجة الحرارة .4- انخفاض درجة الحرارة خلال التخدير الكلى .5- انخفاض درجة الحرارة خلال التخدير الموضعي .6- انخفاض درجة الحرارة المتعمد خلال عمليات القلب المفتوح .7- الزفزفة خلال التخدير وفى فترة ما بعد الجراحة .8- ارتفاع درجة الحرارة .9- العلاقة بين تغيرات درجة حرارة الجسم والأدوية التخديرية بما فى ذلك من مرخيات العضلات .الهدف من العمل:• أن درجة حرارة الجسم تتعرض للعديد من التغيرات فيما قبل وخلال وبعد العمليات سواء خلال التخدير الكلى أو الموضعي .• بعض هذه التغيرات قد تكون معتمدة ( مقصودة ) كما في عمليات القلب المفتوح .• ارتفاع درجة الحرارة قد يحدث فجائيا كما في بعض عمليات الغدة الدرقية .• وأحيانا أخرى قد يحدث انخفاض في درجة الحرارة بدلا من ارتفاعها سواء خلال فترة العملية أو بعدها .وفى هذا البحث سوف نتطرق لـ1- التنظيم الطبيعي لدرجة الحرارة .2- تنظيم درجة الحرارة خلال التخدير الكلى .3- متابعة درجة الحرارة .4- انخفاض درجة الحرارة خلال التخدير الكلى .5- انخفاض درجة الحرارة خلال التخدير الموضعى .6- انخفاض درجة الحرارة فى عمليات القلب المفتوح .7- العلاقة بين تغيرات درجة حرارة الجسم والأدوية التخديرية بما فى ذلك مرخيات العضلات . This study included a total number of 25 patients were divided according to site of A-V fistula into two groups;Group I: included 15 patients; 3 of them were females, 12 of them males. Subdivided according to the duration of haemodialysis (dialytic age) into three groups; Group 1 included 5 patients all of them were males, Group 2 included 14 patients 5 of them were females, 9 of them were males, Group 3 included 6 patients 2 of them were females, and 4 of them were males.And also subdivided according to the age of patients into four groups; Group A included 7 patients: (3 of them were female, 4 of them were male) with age ranged 21 - 30 years. Group B included 5 patients (all of them were male), with age ranged 31- 40 years. Group C included 8 patients (1 of them was female, and 7 of them were males) with age ranged 41 - 50 years.
