Abstract:
Laparoscopic ovarian electrocauterization is considered the 2ndline of treatment in the patients with PCOS. Ovarian drilling byminilaparoscopy under local anesthesia is a new option forgynecologists, allowing similar therapeutical results to those achievedby traditionallaparoscopy, but with benefits of less invasive techniquethat can be carried out in an outpatient service without the need forgeneral anesthesia. Minilaparoscopy required a smaller incision andprovided visualization similar to that of traditional laparoscopy, butthe field of view is slightly narrower than traditional one. Technicalimprovements of small optic systems, and improved anestheticprocedures allow minilaparoscopy to be performed under localanesthesia and conscious sedation. This procedure may become astandard approach for the evaluation of infertile patients with PCOS,in whom ovarian drilling can be integrated into the diagnostic workup.In addition, improvements in sedation and local anesthesiaprotocols, may allow minilaparoscopy to become a routine out patientprocedure that allow cosmetic satisfaction and return of patients tonormal activity also, no need for postoperative narcotic analgesia. Butstill its procedure difficulties need more experience. So, werecommended minilaparoscopy for diagnostic purposes and whenenough experience to be gained shift to operative laparoscopy usingminilaparoscopic techniques may be advised.
