Abstract:
For studying left ventricular diastolic function in ischaemic diabetic patients echo-Doppler examination was performed for 60 subjects classified as follows :Group I : Control group consists of 15 healthy subjects not complaining of ischaemia or diabetes mellitus.Group II : consists of 15 patients complaining of coronary ischaemia and not complaining of diabetes mellitus.Group III : consists of 15 patients complaining of diabetes mellitus (IDDM) and not complaining of myocardial ischaemia.Group IV : consists of l5 patients complaining of ischaemia and diabetes mel1itLls.All the 60 subjects were studied to compare the echoDoppler indices of the patient groups to those of the control group and comparing the Doppler indices of the patient groups with each - other. AbnOl:mal left ventricular diastolic functions were detected in patients compared to the control group.In studying the echo Doppler indices of the ischaemic patients in comparison with the control group we found adecrease in the area under E, mild decrease in the area under A, a decrease In the EI A ratio, non-significant change in the deceleration time and a significant increase in the isovolumic relaxation time in the ischaemic group in comparison with the control group.In studying the echo Doppler indices of the diabetic group 111 compartson with the control group we found a decrease in the peak velocity of E-wave and a significant increase in the peak A-wave and a decrease 111 the EI A ratio, also there IS a significant decrease in the maximum velocity of E-wave and a decrease in the maximum velocity of A-wave. There is also decrease in he deceleration time and a significant increase in the isovolumic relaxation time in the diabetic patients compared to the control group.In studying the echo Doppler indices of the ischaemic diabetic patients compared to those of the control group we found significant decrease of E-wave and a significant increase of A-wave and a significant Increase in the area under Altotal area, also there IS a significant decrease in the ratio of early atrial contraction to the late atrial contraction (EI A ratio).decrease in the deceleration time and a significant Increase In the isovolumic relaxation time IS found 111 the ischaemic- diabetic patients in comparison with the control group.Comparing the echo Doppler indices of the diastolic function in ischaemic patients to diabetic patients no significant changes were found except a significant lI1crease 111 the isovolumic rehixation time in diabetic patients than the ischaemic patients.Comparing the echo Doppler indices of the ischaemic diabetic patients to the ischaemic patients alone and with the diabetic patients alone we found :Significant lI1crease of A-wave.- Significant 1I1crease in the area under A/total area.- Significant decrease in the maximum velocity of E-wave.Significant 1l1crease In the isovolumic relaxation time.- No significant decrease 111 the E/A ratio.- No significant decrease 111 the deceleration time.All 111 the ischaemic diabetic patients compared with ischaemic patients or the diabetic patients by using echo Doppler study of the diastolic function for each group,
