endoscope-aid versus traditional septoplasty

Abstract:
This study was planned to include patients with nasal septum deformity inthe form of spur or posterior deviation. Those patients were calssified intotwo groups.* The 1 sl group ”30 patients” had been operated by traditional septoplastytechnique.* The 2nd group ”30 patients” had been operated by endoscope-aidedseptoplasty technique.Pre-operatively, all patients were subjected to the followings:-* History.* Clinical examination.* Nasal endoscopy and photography.* Rhinomanometry.* Plain x-ray or CT paranasal sinuses.Post-operatively, all patients were subjected to the followings:* Serial clinical examination.* Serial nasal endoscopy and photography.* Serial rhinomanometry.Surgical techniques:(1) Traditiona’ I septoplasty:-Incision at the caudal end of the septal cartilage.- The mucoperichondrial and mucoperiosteal flaps are elevated on bothsides.- If the deformity is in the form of spur it is removed but if in the form ofposterior _ deviation, the spetal cartilage is separated from the vomer and thecartilage is freed from the maxillary crest, then an inferior strip of cartilageis removed if necessary, then the deviated bone of vomer is removed.(2) endoscopic septoplasty:* For spur: with the aid of the nasal endoscope, an incision caudal to thespur is done then the flap is elevated over the spur which is then removed.* For posterior deviation: It follows the same technique as in traditionalone but aided by the endoscope especially in the posterior part.
