Study Of Nitric Oxide State In Chronic Renal Fallure Patients

Abstract:
This work has been done in the Nephrology and Dialysis Unit,Internal Medicine department, Zagazig University hospitals.This study included a total number of 50 subjects, 10 of them asa normal control and the remaining 40 patients were divided accordingto their creatinine clearance into mild, moderate, severe (all of them onconservative treatment) and severe R.F. on regular HD groups.All subjects were subjected to the following:I-Full history taking, and thorough clinical examination.II-Routine laboratory and radiological investigations including:1-Serum creatinine and blood urea levels.2-Liver function tests.3-Total 24 h. urinary proteins.4-Fasting and postprandial blood glucose.5-Serum sodium and potassium.6-Abdominal ultrasonography.III- Renal biopsy in some cases.VI- Special investigations including:1-Plasma levels of combined nitrites and nitrates (NO2 + NO3) =PNOx •2-Urinary levels of combined nitrites and nitrates (2 + NO3) =UNOxWe found that patients with renal failure have increased plasmaNO levels than controls but there was no correlation between itsplasma levels and creatinine clearance.At the same time, there was a significantly lower urinary NOlevels than controls and mild R.F. groups and a positive correlationwith creatinine clearance was found.from this study, we can conclude that:The gradual decrease of urinary NO levels together with thegradual decline in kidney function and without correspondingsignificant gradual increase of its plasma levels suggests that NOproduction is reduced in patients with CRF and CRF is a state of NOdeficiency.
