Urethral spongio-sonography”in cases of anterior urethral stricture ”

Abstract:
To obtain better information on fibrotic tissue depth and length, inthe spongy body in cases of anterior urethral stricture we perfonnedspongiosonography, that is opacification of the spongy body by injectionof contrast medium.The study was undertaken on 26 cases with the average age of 40years, collected from urology department, Zagazig University Hospitaland were divided into 3 groups :Group A : 11 patients with (anterior) urethral stricture.Group B : 10 vasculogenic impotent patients due to venous leakage.Group C : 5 non-vasculogenic impotent patients.Those patient were subjected for the following investigations:urethrogram (antegrade and retrograde), infusion cavernosography,urethroscopy and spongiosography.Spongiosography was done by insertion of 22 gauge butterflyneedle into the glans penis closed to the frenulum, then slowly injection of non-ionic urographic contrast material (300 mg J .Iml iopromide) 1 :1 normal saline (20 cc) over 40 seconds. Then the first film exposed insupine position (anteroposterior), and the second one exposed in obliqueview ( 40 degree).In normal cases : spongiosography showing homogenousopacification of the glans penis, and the spongy body through its wholelength till the bulb of the penis with opacification of the deep dorsal veinof the penis till the santorini plexus.In cases of impotence and anterior urethral stricture,spongiosography showed interrupted opacifications due to venous leakageor multiple urethral strictures respectively.
