Evaluation of computed tomography (ct ) in diagnosis of cholesteatoma and its comparison with operative results

Abstract:
In this study 50, patients who had suffering from secondaryacquired cholesteatomas underwent surgical explorations forremoval of cholesteatomas from the middle ears and mastoids. Everypatient had full medical history general and local examinations,audiometric examinations radiological examination in the form ofplain x-rays and CT examinations using Tomoscan 350 with 1.5 mmcollimation with 0.25 mm2 pixels and extended window width of 3200Hounsfield units in direct axial and coronal planes.Then surgery was done.A comparison between radiological CT finding and operativefinding was done.Radiological versus surgical reports showed the following:Soft tissue masses in middle ear and mastoid:there was an agreement in 92% between CT findings and surgicalfindings.4% of cases showed false + ve.4% of cases showed false - ve.Status of ossicles:There was an agreement in 90% of cases in 2% of cases there was false + vein 8% of cases there was false - ve.Displaced ossicles could be detected on CT without errors.osion of the tegmen:There was an agreement in 95.7% in coronal plane (done for 47es) i.e (45 cases out of 47 cases).1 case had false + ve2.10%As regards the position of the tegmen CT adds nothing more thannormal plain x-ray.:..ateral sinus plate erosion:There was an agreement in 98%only 2% had false - ve.No false + ve.As regards the position of the lateral sinus plate CT adds nothingmore than normal plain x-ray,
