LATE BOOKING FOR ANTENATAL CARE IN M.C.H. CENTERS IN ZAGAZIG CITY

Abstract:
SummeryCare during antenatal period is very important to pregnant mothers as well as babies . Therefore Ministry of Health has established M.C.H centers in every city . At the same time mothers can receive care through the outpatient of the maternity hospitals .Early and regular attendance of the pregnant mother to the antenatal clinic for health supervision is very important, and is imperative for high risk group . late booking was defined as booking for antenatal care after the first trimester .- The aim of the study :-The aim of the study was to determine the magnitude of late antenatal booking, and its causes in M.C.H centers in Zagazig city .- Subjects and methods :-A convenience sample composed of all pregnant mothers attending two M.C.H centers in Zagazig city in a period of 6 months . They were ( 200 ) pregnant mothers ( 50 of them were early bookers and 150 were late bookers ) .-Data were collected using :-An interview questionnaire sheet was used to collect data about demographic characteristics, and socioeconomics condition of the family such as age, education , and occupation of the mother and father , family size , and housing condition . Data about pas- Results revealed the following : -1- The majority of women ( 75 % ) booked late for antenatal care .2- The majority of mothers were housewives ( 94 % ), from urban area ( 92 % of early booking mothers and 88.7 % of late booking group ) , married at age from ( 20 – 25 ) years old , and two thirds of early booking mothers ( 66 % ) were living at middle so3- Half of both groups had a family history of hypertension , liver diseases and diabetes . While one third of late booking group and more than quarter of early booking group had a history of lung diseases , kidney diseases , and gynecological disorders .4- Nearly half of early booking mothers had a short pregnancy space ( less than 2 years ) compared to (31.7% ) of late booking .5- Only ( 17.6% ) and ( 22 . 5% ) respectively of early and late booking mothers had their last delivery with cesarean section .6- Nearly half of both groups were suffering from anemia , hypertension , hypotension and kidney diseases .7- Only ( 8% ) of early booking women didn’t want their pregnancy compared to ( 20% ) of late booking women .8- The highest frequency, and regularity of antenatal visits were among early booking women ( 50% ) compared to ( 38% ) of late booking group .9- More than one third of late booking mothers didn’t come for antenatal care as a result of ignorance about the importance of antenatal care .10- More than half of both groups have got benefitfrom limited cost .While ( 36% ) of both groupshave got benefit from checkup ,and physicalexamination and ( 23.3% ) of late booking motherstook vaccination only .11- Half of early booking didn’t decide about the placeof delivery .SummeryCare during antenatal period is very important to pregnant mothers as well as babies . The head nurse had the highest mean score in both perceived organization and delegation factors ( 5.7 ? 0.9 , 2.3 ? 0.5 ). While the top managers had the hAll perceived time management skills by all levels of nurse managers had higher score than observed one , there was highly significant difference between observed and perceived factors of time management except the comparison of implementation factors betIt was concluded that perception of time management concebt was better than its practice of time management skills among the nurse managers at Zagazig University Hospitals. There was highly significant difference between observed and perceived time manageIt is recommended to revise the undergraduate curriculum to emphasize on time management skills, to conduct orientation programs and in-service education on time management skills, with continuous supervision of nurse manager’s performance.ConclusionIt was concluded form the study that, perception of time management concept by nurse managers at Zagazig University Hospitals was better than their practice which were in unacceptable level. There was highly significant relation between what was observed There was no statistical significant relation between time management skills of nurse managers, and years of their experience or the type of nurse managers’ department. The nurse supervisors had in the observed planning and implementation factors the highAlso, the nurse supervisors had highest mean score ( 8.75 ? 1.5) in the perceived time management skills. The head nurse had the highest mean score in both perceived organization and delegation factors ( 5.7 ? 0.9 , 2.3 ? 0.5 ). While the top managers hadRecommendationsBased on the findings of the study , the following recommendation can be deduced :? Revision of undergraduate curriculum of faculty of nursing, especially nursing service administration course to emphasize the inclusion theory and practice of time management skills.? Staff development programs must be prepared to nurse managers to enhance their applied time management skills, especially the planning activities and delegation skills.? Developing the hospital policies and regulation that figure out the delegation procedures and time management strategies, it should be clear and communicated to nurse managers at all levels.? Orientation programs for the newly employed nurse managers must be planned and implemented including time management training.? Proper communication system must be initiated in Zagazid University Hospitals and maintained through different periodical conferences to discuss and facilitate the exchange of information.? Design nursing formats that facilitate using of time management skills by nurses such as time log, schedule, milestones sheet and grant chart.? Give a chance for ideal role model of nurse managers for helding conferences and meeting e.g agenda, scheduling, prioritizing and delegating.? Design a system for continuous supervision of nurse manager’s performance to ensure proper use of their time and application of delegation skills? Since years of experience of nurse managers not significant relation to time management levels. So , promotion of nurse managers should be based on level of knowledge and competence in application of managerial skills which including time management ski? Developing the nursing records to include format that help nurse managers to manage their time through time loge, schedule, milestones sheet and grant sheet.? Use of advanced technology for computerizing the records, nursing notes and other entries as scheduling which may ultimately save the nurse manager’s time , costs, and increase productivity and the quality of care provided.
